The People Behind the System

The System Behind Continuity of Care

Extending structure, accountability, and performance beyond clinical care-where long-term outcomes are actually built

Gym Training Session
Measure

Track recovery and performance through structured, data-driven benchmarks—creating visibility into consistency, engagement, and real progress.

Engage

Drive accountability and routine through structured programming and on-site implementation—ensuring patients stay consistent beyond clinical care.

Bridge

Bridge clinical care to real-world execution through structured performance, accountability, and continuity

Continuum Performance Systems was created to address a critical gap in behavioral health-what happens after treatment ends.

While most programs provide structure within their walls, maintaining consistency, accountability, and engagement outside of that environment remains a challenge.

CPS was built to extend that structure into real-world settings-where long-term outcomes are actually determined

Why CPS Exists

Traditional treatment models are not designed to support patients beyond discharge in a measurable, structured way.

The result:

  • Decreased engagement

  • Loss of routine

  • Increased risk of relapse

CPS integrates directly into existing treatment frameworks to create continuity-bridging the transition from clinical care to independent living through structured performance programming, accountability, and data-driven tracking.

Mariha Lyons
Founder/Managing Director

Continuum Performance Systems was founded by Mariha Lyons, whose background sits at the intersection of behavioral health, performance, and real-world execution.

With over eight years of experience working within the laboratory and diagnostic space, Mariha has worked closely with treatment facilities across the country-developing a deep understanding of how care is delivered, where gaps exist, and what happens after patients leave structured environments.

Through that exposure, one issue became consistently clear: while clinical treatment provides structure, the transition into real-world environments often lacks the accountability and consistency required for long-term success.

Alongside her work in healthcare, Mariha is an IFBB Wellness Pro and certified personal trainer, bringing a performance-driven approach to structure, discipline, and measurable outcomes. Her background in high-level training and programming is rooted in consistency, execution, and long-term progression-principles that directly translate into behavioral health settings.

CPS was built to apply those principles in a way that extends beyond treatment-creating a system that supports engagement, accountability, and continuity where it matters most.

This is not a generalized wellness approach.
It is a structured, outcome-focused system designed for real-world application.
